Chaperone Signup Form 

Church activities are opportunities for youth to feel the love of the Savior and the influence of the Holy Ghost. While participating, youth need to feel safe and protected.  Leaders and volunteers should follow the Savior’s example of love and concern for children.

As part of church activity safety protocols activities look for a 10 to 1 ration of youth to adult chaperons with always a minimum of two adults in any youth situation.  If you are able to participate as a chaperone, please fill you the form below and an activity leader will be in contact.  If you are able to do more than one activity please specify and you will be contacted as needed.

Please keep in mind youth protection training is required for any volunteers attending youth activities.

What stake do you belong to?

While volunteering to chaperone is not a calling, any adult in contact with youth at church youth events and activities need to have completed youth protection training in the last 3 years.
